Consent to processing and verification
1. By selecting "Continue" and proceeding with this application, the applicant organisation (the "Applicant"), acting through its duly authorised representative, hereby expressly consents, in a manner intended to be legally binding, to the collection, recording, retention, use, disclosure and other processing by Alta Professional Accreditation (the "Accreditation Body") of all information, documentation and data submitted in, with or in connection with this application, together with any further information lawfully obtained in the course of assessment, for the purposes of evaluating the application, determining eligibility for provider approval, administering any approval granted, and exercising the Accreditation Body's legitimate interests in the integrity of its accreditation scheme.
2. The Applicant acknowledges and agrees that the Accreditation Body may, to the extent permitted by applicable law, verify any information provided with third parties, including but not limited to company and charity registries, professional and regulatory bodies, fraud prevention agencies, credit reference agencies, law enforcement bodies and governmental or quasi-governmental authorities, and may disclose information to such parties where reasonably necessary to prevent, detect or investigate misuse, misrepresentation, fraud or other abuse of the service, or where required by law, regulation or order of a competent court or authority.
3. The Applicant warrants that all information and documentation supplied is and shall remain true, complete, accurate and not misleading in any material respect, and undertakes to notify the Accreditation Body in writing without undue delay of any material change. The Applicant is hereby notified that the provision of false, misleading or incomplete information, or the omission of material information, may constitute fraud or a related offence and may be reported to law enforcement and other competent authorities, and shall entitle the Accreditation Body to reject the application or to suspend or withdraw any approval granted.
4. Nothing in this application, nor its submission, acknowledgement or receipt, shall create any entitlement to, or legitimate expectation of, provider approval, and no approval shall be deemed granted unless and until issued in writing by the Accreditation Body. Approval is determined solely by reference to the Accreditation Body's published eligibility criteria and assessment procedures, and the Accreditation Body reserves the right, acting reasonably, to refuse an application or to grant approval subject to conditions.
